EMSA: Sexual Debut, Menstruation and Abortion as topics of sexual and reproductive online health communication

03/2023 - 08/2026
The first time, termination or menstruation are nowadays natural topics in social media – and they are much discussed. However, there is no scientific systematic analysis of posts and public comments in the German-speaking world. For this reason, the Federal Centre for Health Education (BZgA) funds EMSA study research. The study is a quantitative media analysis of posts and public comments in German supplemented by qualitative interviews with adolescents and young adults.

Federal Centre for Health Education (BZgA)

Target group / Sample

Planned target group:

Adolescents and young adults who seek information on sexuality education and sexual health on social media

Objectives

  • Research evaluation of German-language communication in social media on the topics of sexual debut/first sexual intercourse, menstruation and termination of pregnancy/abortion

  • Findings for sexuality education, reproductive health education and advice

  • Research questions:

    • What kind of information do teenagers and young adults find when searching for these topics on social media?

    • How do they deal with this information?

    • How can young people's online health competence be supported?

    • How can professional practice use social media to share evidence-based information?

Methodology / Research design

  • Media content analysis in 3 sub-studies (termination of pregnancy/abortion, the first time, menstruation)

  • Quantitative media content analysis of posts on leading social media platforms (mainly YouTube, TikTok, Instagram)

  • Analysis of the public comments on the corresponding posts

  • Qualitative interviews with adolescents and young adults on the use and subjective effect of social media posts and social media comments on the issues of termination of pregnancy, the first time and menstruation

As part of a communication studies research project, three sub-studies over three years will systematically investigate how

  1. termination of pregnancy/abortion,

  2. the first time and

  3. menstruation

are represented in social media.

The study is based on systematic media content analysis of posts on leading social media platforms (mainly YouTube, TikTok and Instagram). Public comments on the corresponding posts are also evaluated. The quantitative media content analysis is complemented by qualitative interviews with adolescents and young adults on the use and subjective impact of social media posts and comments on the three topics of abortion, the first time and menstruation.

Relevance of the research project

Termination of pregnancy, first sexual intercourse and menstruation are important topics of sexual and reproductive health that are widely presented and discussed in social media. However, German-language posts and public comments on publications on these topics have not yet been systematically scientifically analysed. The EMSA study thus fills a research gap in the field of online health communication.

The study's findings are also relevant for sexuality education and advice services, because only by understanding how abortion, sexual debut and menstruation are presented and discussed on social media today can we reach young people at the level of knowledge and reflection where they currently are.
